Are safety trainers as protective as boots?

At the toe, yes. A safety trainer certified to EN ISO 20345 carries the same 200 joule toecap as a boot. What it does not give you is ankle support, and the upper is lighter. That makes them right for flat, even ground and long shifts on foot, and wrong for broken ground, ladders and anywhere you are carrying awkward loads.

Do they come with a midsole?

Some do. Look for P, PS or PL in the code: those mark penetration resistance, and PS and PL are the 2022 markings for non-metallic midsoles. A trainer with no midsole marking offers no protection against a nail through the sole, which on a construction site is the injury that puts people off work.

Which is more comfortable over a full shift?

The trainer, because it is lighter and the sports last suits walking. Weight matters more than people expect: an extra few hundred grams on each foot is lifted thousands of times a day. If someone is standing rather than walking, cushioning and a supportive insole matter more than the shell.
